Social Services


An important part of each patient’s recovery process is assuring a smooth transition from hospital care to ongoing care and adjustment following discharge.

The Social Services staff works closely with patients, family members and physicians to arrange for home care or nursing home services; to help patients arrange financial assistance when necessary; to arrange transportation or transfer of patients; to provide information on protective and advocacy services; and the formulation of advance directives. Social workers serve as the primary liaison between patients and various support agencies within the patient’s home community.
